The story follows Yanyan (Sue Ramirez), a sunny, optimistic, and endlessly patient woman who works as a caregiver. She takes on a challenging new assignment: to care for Blue (Diego Loyzaga), a wealthy and cynical young man who has earned the nickname “Mr. Heartless.”

However, his nickname has a tragic double meaning. Not only is he emotionally cold and pushes everyone away, but he is also literally “heartless”—he suffers from a severe heart condition that has left him frail, bitter, and resigned to an early death.

Yanyan’s job becomes twofold: she must manage his physical health while also trying to break through the emotional walls he has built as a defense mechanism. At first, he is cruel and dismissive, determined to get rid of her like all the caregivers before her. But her persistent kindness and refusal to give up on him slowly begin to melt his icy exterior.

As Blue starts to let Yanyan into his life, they begin to fall for each other. He finds a reason to want to live, and she finds a love that she is desperate to protect. The central conflict of the film becomes a poignant question: can her love truly heal him, or is their budding romance destined for tragedy due to his failing heart?
